MSK Access — Bone, Joint & Spine Care Through the CMS ACCESS Model
MSK Access is a personalized care program for bone, joint, and spine health, now enrolling Medicare members through the CMS ACCESS Model. Move strong again, and get back to what you love.
What members get
- A self-managed care plan generated and tailored to you, that adjusts as you improve.
- A 15-minute daily home exercise plan tailored to your body, your pain, and your pace.
- Protocols built with input from physicians and physical therapists.
- Your physician or physical therapist stays in the loop on your progress.
- Made for knee pain, hip pain, back pain, arthritis, and recovery after injury or surgery.
- Help finding follow-up with trusted in-person physical therapy partners near you when hands-on care helps.
The CMS ACCESS Model
The ACCESS Model is a Medicare program from the Centers for Medicare & Medicaid Services (CMS) built to give people earlier, easier access to high-quality musculoskeletal (bone, joint, and spine) care. MSK Access participates in the model, which is how eligible Original Medicare members join the program at no cost. A care coordinator confirms coverage before anyone starts. Medicare Advantage coverage is expected in 2028.
Bone health: prevent the next fracture
A broken bone after 50 is usually a sign your bones need attention, and most people are never evaluated afterward. The MSK Access bone health track includes a guided review of fracture risk and bone density history, evidence-based strength and balance training, practical fall prevention coaching, and a flag for you and your doctor when medication or testing may be worth discussing.
How it works
- Check your eligibility — a few questions, about two minutes.
- Start your care — a self-managed care plan generated and tailored to you.
- Move strong again — a daily plan that grows with you, your doctor kept in the loop, and help finding a physical therapist nearby when you need hands-on care.
In-person care partners
When a plan calls for hands-on care, MSK Access connects members with physical therapy partners in their community, including Duke Health (North Carolina), Team Rehabilitation Physical Therapy (Michigan, Illinois, Indiana, Wisconsin, Georgia), Alliance Physical Therapy Partners (275+ clinics nationwide), Western Rehabilitation Health Network (Idaho, Utah, and eastern Oregon), Mountain Land Physical Therapy (Utah, Idaho, and Montana), Dry Creek Physical Therapy (Utah), Therapydia (Oregon and Washington), Keystone Spine and Pain Management Center (Pennsylvania), Louisville Institute of Orthopaedics (Kentucky), and Med First Primary & Urgent Care (North Carolina and South Carolina). Footprints are shown from partner rosters and public materials.
Trusted resources
MSK Access works alongside patient education from Own the Bone (American Orthopaedic Association), the National Spine Health Foundation, and HipKneeInfo (American Association of Hip and Knee Surgeons).
For organizations
PT groups, orthopedic practices, ACOs, and health systems can partner with MSK Access on the CMS ACCESS Model — including co-management revenue of up to $100 per enrolled beneficiary in year one ($90 ongoing per year, per MSK track) and an interactive ROI calculator.
